INDIVIDUAL ORDER FORM
Moscow Ballet is offering discounted ticket rates to groups. Two options are available:
1) Individuals simply return their completed order forms to your ticket coordinator with payment. Your coordinator then completes the Group Ticket Order Form and sends form to Moscow Ballet with one company check made payable to: Nut 2002, Inc. or return by fax with credit card number and signature (Mastercard, Visa, American Express & Discover accepted).
2) Individuals call local ticketing agency (# listed below). State that you want tickets to the Moscow Ballet's "Great Russian Nutcracker" as well as date, time and location desired. Request B-type ticket in order to receive discounted rates. Be aware that the ticketing agency's service charge and handling fee will apply.
